Quick Answer: Neither type universally wins for back pain. Memory foam excels at pressure relief; hybrid mattresses provide stronger lumbar support through their coil base. Research points to medium-firm as the most consistently helpful for back pain. Your body weight and sleep position determine which construction gives you that medium-firm feel. Most back pain sufferers do best on a hybrid or quality pocket coil mattress that combines support with comfort layer cushioning.

What Your Mattress Does to Your Spine
A mattress affects your back in two ways: it provides support for your spine's natural alignment, and it provides pressure relief for your hips and shoulders. The tension between these two requirements is at the heart of the hybrid versus memory foam debate for back pain sufferers.
If a mattress is too firm, it does not give way under your hips and shoulders. Your spine ends up in a slightly arched or misaligned position to compensate. If it is too soft, your hips sink too deeply, which also throws your spine out of alignment -- just in the opposite direction. The goal is a surface that supports the natural curvature of your spine while cushioning the pressure points that bear the most weight.
The alignment equation. For a side sleeper, the ideal mattress allows the hip and shoulder to sink in just enough that the spine runs parallel to the mattress surface. For a back sleeper, it supports the lumbar curve without letting the lower back sag. Stomach sleepers (generally discouraged for back pain sufferers) need a firmer surface to prevent the pelvis from sinking and hyperextending the lumbar spine.

What the Research Actually Says
The most-cited mattress research on back pain is a 2008 study by Jacobson et al. published in Applied Ergonomics. It found that new medium-firm mattresses significantly reduced back pain and improved sleep quality compared to the participants' old mattresses. A 2015 study in The Lancet found medium-firm mattresses outperformed firm mattresses for chronic non-specific low back pain.
What neither study resolved is whether the medium-firm benefit comes from a specific mattress type -- spring, foam, or hybrid. The firmness level appears to matter more than the material achieving that firmness. This nuance is important because it means there is no universally superior material for back pain. The question becomes: which construction is most likely to provide and maintain a medium-firm feel for your specific body?
Memory Foam and Back Pain
Memory foam's pressure-relieving properties are well-suited for one component of back pain management: cushioning the hips and shoulders so they don't bear concentrated pressure points. Side sleepers with hip or shoulder pain from their mattress often find significant relief on memory foam.
The potential weakness of memory foam for back pain is lumbar support, particularly for back sleepers. Dense memory foam contours closely to the body, which means it can allow the lower back to sink slightly deeper than ideal. Stomach sleepers on memory foam are most at risk of this -- the pelvis sinks into the foam, creating a pronounced lower back arch.
Foam firmness degrades over time. A medium-firm memory foam mattress may provide excellent support initially and gradually soften as the foam compresses. Lower-density foam (under 4 lb per cubic foot) can soften noticeably within 3-4 years. If your back pain has worsened on a mattress you've had for several years, foam softening is a likely contributor.
Higher-density memory foam and zoned memory foam mattresses address the lumbar support concern more effectively. Zoned construction uses firmer foam in the lumbar zone and softer foam at the shoulders and hips. The Restonic Luxury Silk & Wool at Mattress Miracle uses 884 zoned coils in a queen -- zoned construction is not unique to foam, and in coil systems it is achieved by varying coil gauge or density across different mattress zones.
Hybrid Mattresses and Back Pain
Hybrids combine a pocket coil support core with foam comfort layers. For back pain sufferers, this architecture has specific advantages. The coil system provides firmer, more consistent lumbar support that doesn't compress the same way foam does. The foam comfort layers above it still provide pressure relief at the hips and shoulders.
The result is a mattress that can achieve the medium-firm feel research supports while distributing pressure relief and structural support more effectively than an all-foam construction. For back sleepers and stomach sleepers especially, the firmer coil base tends to maintain better spinal alignment.
Dorothy's assessment. In years of helping Brantford customers with back pain, the most common outcome is that people who had been sleeping on a worn, sagging mattress find significant improvement on almost anything newer and properly supportive. Among new mattresses, we tend to guide back-pain customers toward pocket coil options with medium-firm specifications, because the coil support holds up long-term and doesn't degrade the way foam softens. The Restonic ComfortCare at $1,619 for a queen is our most frequently recommended starting point for this concern.
How Sleep Position Changes the Equation
Sleep position significantly shifts which construction performs better for back pain:
Side sleepers with back pain generally benefit from more pressure relief at hips and shoulders. Memory foam or a hybrid with a substantial foam comfort layer works well. A too-firm surface causes hip and shoulder pressure that translates to spinal misalignment and wakes the back up sore.
Back sleepers with lower back pain need lumbar support combined with mild cushioning. Hybrids tend to provide better consistent support for back sleepers over time, because the coil system doesn't degrade the same way a foam support core might.
Stomach sleepers (generally not recommended for back pain) need firm support to prevent the pelvis from sinking. Hybrid or a firm spring mattress is usually better than memory foam for this position, as memory foam tends to contour under the pelvis and create lumbar hyperextension.
Why Body Type Matters More Than Mattress Type
A 150 lb person and a 250 lb person lying on the same mattress have dramatically different experiences. The 150 lb person may find the mattress medium-firm; the 250 lb person may find it too soft, as their greater weight causes the comfort layers to compress more deeply.
This means the same mattress can help one person's back pain and worsen another's. What reads as "medium-firm" on a specification sheet is only meaningful relative to the body weight applying pressure to it. Heavier sleepers generally need a firmer base to achieve the same effective support level as lighter sleepers on a medium mattress.
Comparison: Hybrid vs Memory Foam for Back Pain
| Factor | Memory Foam | Hybrid |
|---|---|---|
| Pressure relief | Excellent | Good to Very Good |
| Lumbar support (back sleepers) | Good (firm foam) to Fair (soft foam) | Very Good (coil base) |
| Long-term support durability | Moderate (foam degrades) | Good to Very Good (coils hold firm) |
| Best for sleep position | Side sleepers | Back and combo sleepers |
| Best for body weight | Light to average (under 200 lb) | Average to heavier (160+ lb) |
| Temperature | Runs warm | Cooler (coil airflow) |
Options at Mattress Miracle
For customers dealing with back pain, we walk through sleep position, body weight, and what their current mattress is doing wrong before landing on a recommendation. The Restonic ComfortCare line, available in multiple firmness levels from medium to firm, is our most common recommendation. The 1,222 individually wrapped coils in a queen provide consistent support across the mattress surface, including the critical lumbar zone.
The Restonic Luxury Silk & Wool (queen at $1,395) uses 884 zoned coils specifically designed to provide targeted support -- firmer in the centre third where the lower back needs it most, and more responsive at the head and foot zones. This zoning is genuinely useful for back-pain sufferers, not just a marketing feature.
Dorothy's Note: The most important thing I tell customers with back pain is to spend real time in the showroom. Lie on each mattress in your actual sleep position for at least 5 minutes. Don't sit on the edge and bounce -- that tells you nothing. A mattress that relieves your back pain will feel supportive but not stiff, and you should feel your lower back resting comfortably, not floating away from the surface.

Is memory foam or a hybrid better for lower back pain?
It depends on your sleep position and body weight. For side sleepers, memory foam's pressure relief is valuable. For back and stomach sleepers, the firmer coil support of a hybrid tends to provide better lumbar support that holds up over time. Heavier sleepers (180 lb+) often find hybrids provide more consistent support than memory foam, which can soften excessively under greater weight.
What firmness is best for back pain?
Multiple studies point to medium-firm as the most consistently helpful for back pain. This is roughly a 5-7 on a 1-10 firmness scale. Critically, "medium-firm" is relative to your body weight -- a heavier person needs a firmer mattress to achieve the same effective support level that a lighter person gets from a medium mattress.
Can a mattress topper help with back pain if I can't replace my mattress?
Sometimes. If your current mattress is too firm and causing pressure-point pain at hips and shoulders, a softer foam topper can relieve the pressure. If your mattress is too soft and sagging, a firm latex topper can add some support back. However, a topper cannot fix a significantly worn or sagging mattress -- it will just follow the sag.
How quickly should a new mattress relieve back pain?
Most people notice a meaningful difference within the first two weeks. It can take up to 30 days to fully adjust to a new sleep surface and for your body to respond. If back pain persists after 4 weeks on a new mattress, consult a physiotherapist or doctor -- the mattress may not be the primary cause.
