Why Your Shoulder Hurts From Sleeping

The shoulder joint has the greatest range of motion of any joint in the body, which also makes it the most vulnerable to compression injury. When you sleep on your side (the most common position), your entire upper body weight presses through the shoulder into the mattress for 7-8 hours. A mattress that does not adequately cushion this area creates:
- Impingement: The rotator cuff tendons get compressed between the humeral head and acromion. Hours of compression causes inflammation and morning pain
- Reduced blood flow: Sustained pressure restricts circulation to the joint, causing numbness, tingling, and stiffness upon waking
- Aggravation of existing conditions: Rotator cuff injuries, bursitis, frozen shoulder, and arthritis are all worsened by hours of compression on a hard surface
What to Look For in a Mattress
Shoulder Pain Mattress Features
Thick comfort layer (3-4+ inches): This is the most important feature. A thick layer of memory foam, latex, or soft polyfoam allows the shoulder to sink deep enough to relieve pressure. Thin comfort layers (1-2 inches) are insufficient for most side sleepers with shoulder pain.
Zoned support: Some mattresses have softer zones at the shoulders and firmer zones at the hips and lumbar. This is ideal for shoulder pain because it provides targeted relief without sacrificing spinal alignment.
Medium to medium-soft firmness (4-6): Firm mattresses are the most common cause of shoulder pain from sleeping. If you switched to a firm mattress and developed shoulder pain, the mattress is very likely the cause.
Resp
onsive support core: Individually wrapped coils (hybrid) or high-density foam that supports the body while the comfort layer cushions the shoulder. Zoned coils with a softer shoulder section are the premium option.
Recommendations by Shoulder Condition
- Rotator cuff injury/surgery: Medium-soft with thick memory foam comfort layer. An adjustable base helps you find the least painful position. Avoid sleeping on the injured side if possible. A body pillow prevents rolling onto the injured shoulder
- Bursitis: Pressure is the enemy. Choose the softest mattress that still supports your spine. Memory foam excels here for its zero-pressure-point design
- Frozen shoulder (adhesive capsulitis): Medium with good support. Frozen shoulder needs a mattress that allows some sinking but does not create positions where the arm gets trapped. A pillow in front to rest the affected arm helps
- Arthritis: Medium with memory foam or latex. Consistent cushioning reduces the morning stiffness associated with inflammatory conditions. Temperature regulation helps too (cold can worsen arthritis stiffness)
- General shoulder pain (no diagnosis): If shoulder pain occurs only upon waking and resolves within an hour, your mattress or pillow is almost certainly the cause. A medium mattress with 3+ inches of memory foam comfort layer typically resolves this
Complete Shoulder-Friendly Sleep Setup
Beyond the Mattress
Pillow: A pillow that properly fills the shoulder-ear gap prevents the neck from tilting, which reduces the angle of shoulder compression. Too thin = neck drops = more shoulder load. (2) Arm position: Do not sleep with your arm under the pillow or under your body. Keep the arm in front at
a natural angle, resting on a pillow if needed. (3) Alternate sides: If possible, switch which side you sleep on to distribute the compression. (4) Mattress topper: If buying a new mattress is not feasible immediately, a 3-4 inch memory foam topper on your existing mattress can provide significant shoulder pressure relief for $100-200. (5) Gentle morning stretches: A few minutes of shoulder mobility exercises after waking helps blood flow return and reduces stiffness.

Can a mattress cause shoulder pain?
Absolutely. A mattress that is too firm is one of the most common causes of shoulder pain, especially in side sleepers. If your shoulder pain started after getting a new mattress, or if it only occurs after sleeping and resolves during the day, the mattress is very likely the cause.
Should I sleep on my back to avoid shoulder pain?
Back sleeping eliminates shoulder compression entirely. If you can sleep on your back, it is the best position for shoulder problems. Use a thin pillow and consider a small rolled towel under the neck for support. However, many people cannot comfortably switch to back sleeping, and side sleeping is fine with the right mattress.
Will a mattress topper help shoulder pain?
Yes, if the issue is a too-firm mattress. A 3-4 inch memory foam topper adds the cushioning layer your shoulder needs. It is a good interim solution while you decide on a new mattress. Choose medium or medium-soft density for shoulder relief.
How long before a new mattress helps shoulder pain?
Most people notice improvement within the first week. Significant improvement (less morning stiffness, less pain) typically occurs within 2-4 weeks. If a new, properly-chosen mattress has not helped after 30 days, the shoulder pain may have a medical cause beyond mattress pressure.
