Quick Answer: To remove bed bugs on a mattress, vacuum seams and surfaces thoroughly, apply direct steam along all seams, and encase the mattress immediately in a bed bug-proof encasement. For anything beyond surface-level bugs, professional heat treatment is the only reliable method that reaches the mattress interior where eggs and hiding bugs reside.

Where Bed Bugs Hide on a Mattress
Understanding where bed bugs actually live on a mattress shapes which treatment methods matter. Bed bugs are flat and can squeeze into very narrow spaces, they prefer tight harbourages near their food source (you).
Common Bed Bug Locations on a Mattress
- Seams and welting tape: The piped edging along all four sides is a primary hiding spot, check carefully with a torch
- Tufts and button recesses: Any depression or fold in the mattress surface is a preferred hiding location
- Label area: The fabric tag and surrounding area on the bottom is commonly infested
- Box spring underside: The gauze fabric on the underside of box springs is a very common harborage
- Mattress interior: In established infestations, bugs and eggs penetrate through seams into the foam or batting

Surface Treatment: Vacuuming and Steam
For early-stage infestations with bugs visible primarily on the mattress surface and seams, two physical methods are effective.
Thorough Vacuuming
Use a vacuum with a HEPA filter and the narrow crevice tool. Work slowly along every seam, tuft, and fold. Apply firm pressure. Go over each area at least twice. Immediately after vacuuming, remove the vacuum bag, seal it in a plastic bag, and take it outside to the rubbish, bed bugs can escape an unsealed vacuum. This removes visible adults, nymphs, and some eggs but will not reach bugs inside the mattress.
Steam Treatment
A steam cleaner produces temperatures well above the 49°C lethal threshold on the surface it contacts. Run the steam wand slowly along all seams, tufts, and the mattress surface, approximately 3–5 cm per second to allow the heat to penetrate. Steam kills bugs on contact. It does not penetrate deeply into the mattress interior. Used after vacuuming, it addresses the surface bugs that vacuuming missed.
After Vacuuming and Steaming
After the vacuum-and-steam treatment, allow the mattress to dry completely before encasing (steaming introduces some moisture to the surface). Then immediately encase in a bed bug-proof mattress encasement. This sequence, vacuum, steam, encase, is the most thorough DIY surface treatment available.
Encasing the Mattress
A bed bug-proof encasement should be applied after initial treatment and left in place for 18–24 months. It does three things: prevents new bugs from accessing the mattress as a harborage; traps any remaining interior bugs until they starve; and gives you a clean surface to inspect, any new bugs caught on the encasement surface are coming from elsewhere in the room, telling you the infestation hasn't been fully resolved.
For the encasement to work, it must be completely intact, no tears, no open seams, no broken zipper. Inspect it monthly. Some encasements come with a zipper guard to prevent bugs from escaping through the zipper track.
Interior Infestation: Beyond DIY
Vacuuming and steam address surface and seam bugs. They cannot reach bugs and eggs that have penetrated into the mattress core. If you find evidence of bugs deep in seams or the infestation is more than a few weeks old, assume the interior is also infested.
At this stage, the options are:
- Professional heat treatment: The mattress is heated to lethal temperature internally, the only reliable method for killing interior bugs and eggs without destroying the mattress
- Mattress replacement: Combined with immediate encasement of the new mattress and treatment of the rest of the room
Attempting to continue DIY treatment for an established interior infestation typically results in months of incomplete control and ongoing bites. Professional treatment is more efficient and reliable.
Keep or Replace the Mattress?
The decision depends on the infestation severity and the mattress condition:
| Situation | Recommendation |
|---|---|
| Early, surface-level infestation; mattress in good condition | Treat and encase, don't replace |
| Established infestation; mattress still supportive | Professional heat treatment; then encase |
| Established infestation; mattress already old or worn | Replace; encase new mattress immediately |
| Severe infestation with visible structural damage to mattress | Replace; seal and mark old mattress before disposal |
Brad, Owner, 40+ years of experience: "When someone calls about replacing a mattress after bed bugs, I always ask: was the mattress due for replacement anyway? If it's 10 years old and you've been through a bed bug situation, this is as good a time as any to start fresh. But if the mattress is still in good shape, a proper heat treatment means you don't have to throw it out."

Can bed bugs live inside a mattress, not just on the surface?
Yes. In established infestations, bed bugs and eggs penetrate through seams and tufts into the mattress interior. This is why surface treatments like vacuuming and steam, while useful, are insufficient for complete elimination. Professional heat treatment that raises the entire mattress to lethal temperature is required to address interior bugs.
Does steam kill bed bugs on a mattress?
Steam kills bed bugs on contact when applied at temperatures above 49°C. It's effective for surface and seam bugs when applied slowly (3–5 cm per second). Steam does not penetrate deeply into the mattress interior, so it complements but does not replace thorough vacuuming and professional treatment for established infestations.
How long does a mattress encasement need to stay on for bed bugs?
Bed bugs can survive up to 18 months without feeding. The encasement must remain intact and undamaged for at least 18–24 months to ensure all trapped bugs have died from starvation. Check it monthly for tears or damage at seams and the zipper area.
