When anxiety triggers chest pain, people understandably jump to the worst conclusion. But anxiety chest pain has specific, well-understood physical mechanisms. Understanding what is actually happening in your body removes some of the fear, which itself helps reduce the pain.
1. Intercostal Muscle Tension
The muscles between your ribs (intercostal muscles) contract during the fight-or-flight response. When anxiety persists for hours or days, these muscles can develop sustained contractures, similar to a cramp. This produces a sharp, localised pain that can feel alarming but is muscular in origin. You might notice it worsens when you twist your torso or press on a specific spot on your chest wall.
2. Hyperventilation and CO2 Depletion
Rapid, shallow breathing during anxiety reduces carbon dioxide levels in your blood by up to 30 percent. This causes blood vessels to constrict, including those in the chest wall. The resulting reduced blood flow to chest muscles produces a tight, constricting sensation. It also causes tingling in the hands and face, dizziness, and a feeling of breathlessness that perpetuates the anxiety cycle.
3. Adrenaline-Driven Coronary Artery Spasm
This mechanism is less discussed but clinically significant. During intense anxiety, the surge of adrenaline and cortisol can cause brief spasms in the coronary arteries. These transient spasms reduce blood flow to the heart muscle momentarily, producing genuine cardiac-type discomfort. A study published in Primary Care Companion to the Journal of Clinical Psychiatry noted that patients with panic disorder showed higher rates of coronary artery spasm on testing (Fleet et al., 2000).

Should I take aspirin during anxiety chest pain?
If you are unsure whether chest pain is anxiety or cardiac, taking one regular aspirin (325 mg, chewed) while calling 911 is standard first aid advice. Aspirin will not help anxiety-related chest pain, but it can be life-saving during a heart attack by inhibiting blood clotting. The downside of taking aspirin unnecessarily is minimal.
