Quick Answer: Bed bug mattress spots come in two forms: dark brown/black fecal dots (1-2 mm, ink-like, bleed when dabbed with a wet cloth) and rust-red blood stains (from crushed bugs or bites). They cluster along mattress seams, piping, and near the headboard end. If you find these spots, inspect for live bugs, encase the mattress, and contact a licensed pest control professional.

Three Types of Bed Bug Spots on a Mattress
Bed bugs leave behind three distinct types of evidence on mattress surfaces. Each one looks different and tells you something specific about the infestation.
1. Fecal Spots (Dark Brown to Black Dots)
These are the most reliable indicator. Bed bug fecal matter is digested blood, and it appears as small, dark dots about 1 to 2 mm across. They look remarkably similar to ink dots from a fine-tip pen. When fresh, fecal spots are slightly raised and wet. As they dry, they flatten and become absorbed into the fabric, leaving a permanent dark stain.
The key identifying feature: fecal spots bleed when touched with a damp cloth. If you press a wet cotton swab or white cloth against a suspected spot and it smears into a reddish-brown streak, it is almost certainly bed bug excrement. This "smear test" is one of the simplest ways to confirm bed bug activity on a mattress.
2. Blood Stains (Rust-Red Smears)
Blood stains on a mattress come from two sources. The first is a crushed bed bug. If you roll onto a recently fed bug during the night, the blood it consumed (your blood) is released onto the sheet or mattress surface. These stains are rust-red and often slightly smeared, as if something was pressed and dragged across the fabric.
The second source is the bite wound itself. Bed bug bites can continue to bleed slightly after the bug has finished feeding. Small dots of bright red blood on the sheet, often in a line or cluster, may indicate active biting.
Blood stains alone do not confirm bed bugs. A scratch, a nosebleed, or a minor cut can leave similar marks. Blood stains become significant when combined with fecal spots or other bed bug evidence.
3. Shed Skins (Translucent Husks)
Bed bugs moult five times before reaching adulthood. Each moult leaves behind a translucent, amber-coloured shell that looks like a hollow version of the bug. These skins are not technically "spots," but they are often found alongside fecal stains in the same seam areas. Finding shed skins confirms that bed bugs have been living and growing in or near the mattress.
| Evidence Type | Appearance | Size | Location | What It Confirms |
|---|---|---|---|---|
| Fecal spots | Dark brown/black dots | 1-2 mm | Seams, piping, crevices | Active feeding and hiding nearby |
| Blood stains | Rust-red smears or dots | 2-10 mm | Top surface, sheets | Recent feeding (possible, not definitive) |
| Shed skins | Translucent amber husks | 1-5 mm (varies by stage) | Seams, behind tags, box spring | Established, growing population |

The Wet Cloth Test
This is the simplest at-home test to distinguish bed bug fecal spots from other dark stains on a mattress.
- Take a white cloth or cotton swab and dampen it with water.
- Press the damp cloth firmly against the suspected spot for 10 seconds.
- Lift the cloth and examine it.
If the spot transfers a reddish-brown streak onto the white cloth, it is consistent with bed bug excrement. Bed bug feces are composed of digested blood, and the iron in the blood reacts with moisture to produce that characteristic rust-brown colour. Dirt, mould, or ink will either not transfer or will transfer in a different colour.
This test is not 100% definitive on its own, but combined with the spot's location (mattress seams) and pattern (clustered dots), it provides a strong indication.
Dorothy, Sleep Specialist: "We get calls from customers who are worried about dark spots on their mattress. About half the time, it turns out to be something harmless, like dirt from a pet, a spilled drink, or fabric dye transfer from sheets. The wet cloth test is the quickest way to narrow it down before calling an exterminator."
Bed Bug Spots vs. Other Common Mattress Stains
Not every dark spot on a mattress is cause for alarm. Here is how to tell the difference.
| Stain Type | Colour | Pattern | Wet Cloth Test | Other Clues |
|---|---|---|---|---|
| Bed bug fecal | Dark brown/black | Clustered dots along seams | Reddish-brown smear | Often with shed skins nearby |
| Mould/mildew | Green, grey, white, black | Irregular patches | No colour transfer | Musty smell, fuzzy texture |
| Sweat stains | Yellow to light brown | Large diffuse areas | No colour transfer | Body-shaped, gradual |
| Rust from frame | Orange-brown | Streaks or ring patterns | Orange-brown transfer | Located on bottom of mattress |
| Ink or dye transfer | Blue, black, or coloured | Random placement | Matches the ink/dye colour | Flat, no texture |
| Food or drink spills | Varies | Irregular shape | No reddish-brown | May have residual odour |
The most commonly confused stain is rust from a metal bed frame. Rust spots can look similar to bed bug blood stains, but they are typically on the underside or edges of the mattress where the frame contacts the fabric, not along the top-surface seams where bed bugs congregate.
Where Spots Appear on a Mattress
Bed bugs are creatures of habit. They feed at night and retreat to nearby hiding spots during the day. On a mattress, this means the spots follow predictable patterns.
Highest Concentration Areas
- Along the mattress piping on the top surface, especially within arm's reach of where you sleep. This is the most common location for fecal spots.
- Mattress seams at the head end. Bed bugs prefer to be close to the head and shoulders because they are attracted to the carbon dioxide you exhale.
- Under the mattress tag or label. The fabric overhang of a sewn-on tag provides shelter.
Secondary Areas
- Box spring fabric, especially the underside. Bed bugs often establish larger populations on the box spring than the mattress itself because the fabric is less disturbed.
- Bed frame joints and headboard crevices. Any crack wide enough to slide a credit card into is wide enough for a bed bug.
- Wall behind the headboard. Fecal spots on the wall above or behind the headboard are a sign of a well-established infestation.
Distribution Research
A study published in the Journal of Medical Entomology examined bed bug harbourage distribution in infested apartments and found that 70% of bed bugs in a room were within 1.5 metres of the bed, with the mattress and box spring harbouring the largest concentration. Fecal spotting was most dense along mattress seams and box spring edges, confirming these as the primary inspection points (Wang et al., Journal of Medical Entomology, 2010).
What the Spots Tell You About the Infestation
The quantity and distribution of bed bug mattress spots can give you a rough idea of how established the infestation is.
A Few Scattered Spots
A small number of fecal dots (under 10) in one area of the mattress seam, possibly with one or two blood spots on the sheets, suggests an early-stage infestation. You may be dealing with just a few bugs. This is the best time to act because treatment is faster, cheaper, and more likely to succeed in a single round.
Clusters Along Multiple Seam Sections
Fecal spots in multiple locations along the mattress piping, combined with shed skins, indicates a population that has been growing for weeks to months. At this stage, there are likely dozens of bugs in and around the bed. Professional treatment is necessary.
Heavy Staining on Mattress, Box Spring, and Walls
Dark staining across large areas of the mattress, visible spots on the box spring and nearby walls, and a noticeable sweet, musty odour indicate a severe infestation with hundreds of bugs. Multiple treatment rounds will likely be needed, and replacing the mattress may make more sense than attempting to clean and salvage it, especially if the mattress is already several years old.
Brad, Owner since 1987: "When someone comes in after a bed bug situation, I never make them feel bad about it. It happens to anyone. What I care about is making sure they get a fresh start. If the mattress can be saved with a good encasement after treatment, great. If it is too far gone, we find them something new that fits their budget and get it delivered once the exterminator gives the all-clear."
Next Steps If You Find Bed Bug Spots
Finding bed bug mattress spots is stressful, but the situation is manageable with the right approach.
- Confirm the identification. Use the wet cloth test on fecal spots. Look for additional evidence: live bugs, shed skins, or eggs in mattress seams.
- Do not move the mattress. Dragging it through the house spreads the infestation.
- Strip and hot-wash all bedding. Wash at 60°C minimum, dry on high heat for 30+ minutes.
- Install a bed bug encasement. This traps bugs inside and prevents new ones from colonizing the mattress. A proper encasement has a secure zipper with bug-proof closure, not a standard mattress protector.
- Contact a licensed pest control professional. In Ontario, verify they hold a valid license from the Ministry of the Environment. Ask about heat treatment, which kills all life stages in a single session.
- After treatment, keep the encasement on for 12 months. Bed bugs can survive months without feeding, so the encasement needs to stay sealed to ensure all trapped bugs die.
If the mattress needs replacing after treatment, wait until your pest control professional confirms the infestation is eliminated. There is no point putting a new mattress into a room that still has live bugs. Our mattress lifespan guide can help you decide if replacement makes sense based on the age and condition of your current bed.

What colour are bed bug stains on a mattress?
Bed bug fecal spots are dark brown to black, about the size of a pen dot, and bleed slightly when wet. Blood stains from crushed bed bugs are rust-red to reddish-brown. Fresh blood spots may appear bright red but darken to a rusty colour within hours. Both types of stains are permanent and do not wash out completely.
Can bed bug stains be removed from a mattress?
Partially. Fresh stains can be lightened with cold water and an enzymatic cleaner or hydrogen peroxide, but older bed bug fecal stains permanently discolour fabric. The stain removal is cosmetic and does not address the infestation. Focus on treating the bed bugs first, and deal with stain removal afterward.
How do I tell the difference between bed bug spots and mould?
Bed bug fecal spots are flat, smooth, and ink-like. They are round dots about 1-2 mm across. Mould spots are usually fuzzy or textured, may have a green, grey, or white tint, and often have an irregular shape with a musty odour. Bed bug spots do not smell on their own, though heavy infestations can produce a sweet, musty odour from the bugs themselves.
Do bed bug spots only appear on the mattress?
No. Bed bug fecal spots and blood stains can appear on sheets, pillowcases, the box spring, the bed frame, the wall behind the headboard, and even on curtains or furniture near the bed. If you find spots on the mattress, check all surrounding surfaces as well.
