When the immune system detects an invader, it releases signalling molecules called pyrogens. These reach the hypothalamus and raise the body's temperature target -- its set point -- upward, often by 1 to 3 degrees Celsius. At that moment, your actual body temperature is below the new target, so the body treats itself as "too cold." It responds by generating heat through shivering and peripheral vasoconstriction. You feel cold even though you are not: this is the chill phase.
As temperature rises to meet the new set point, the shivering stops. If the immune system then achieves its goal -- neutralising the pathogen -- the set point resets back down. Now your core temperature is above the reset target, and the body must shed heat rapidly. Vasodilation and sweating follow. This is the sweat phase.
The entire cycle can occur multiple times per night in certain infections. Malaria classically produces cycles of cold-chill-sweat that correlate with parasite replication cycles. Bacterial infections can produce similar patterns with less precise timing.
Chills and Sweats Without Measurable Fever
