During the day, gravity does a lot of work keeping stomach contents where they belong. When you are upright, acid that escapes the lower esophageal sphincter (LES) falls back down quickly. Lying flat removes that gravitational advantage.

Sleep also reduces swallowing frequency. During waking hours, you swallow about 25 times per hour, each time clearing acid from the esophagus and coating it with saliva that neutralizes acid. Asleep, you swallow maybe 4-5 times per hour. The natural clearing mechanism slows significantly.

The result: acid stays in contact with the esophageal lining longer, causing more irritation and the burning sensation that wakes people at 2 or 3 a.m.

Best Sleep Position for Reflux
