Quick Answer: The best mattress for back pain is usually medium-firm with firmer support under the lower back. A 2003 Lancet trial of 313 adults with chronic low-back pain found medium-firm beat firm at 90 days for pain in bed and disability. Its 90-day result for pain on rising was not statistically clear. New leg numbness or weakness needs a doctor first.

A mattress for back pain is one whose support core keeps your spine straight while you sleep. On top, it needs enough cushion for your shoulders and hips to settle in. Firmness is how the top layer feels; support is whether the middle of the bed stays level under your hips. At Mattress Miracle, a family-owned store in Brantford since 1997, the research below points most people with a sore back to our medium-firm mattresses for back pain. That collection starts at $540, the sale price of the Pocket Care Premium twin (regularly $679).
Medium-firm has a precise meaning in the research. The 2003 Kovacs trial in The Lancet rated mattresses on the European Committee for Standardisation H(s) scale, where 1.0 is the firmest and 10.0 the softest. Its firm mattresses scored 2.3 and its medium-firm ones 5.6. Store scales usually run the other way, with 10 as the firmest, so one medium-firm bed can be labelled a 5, a 6 or a 7. Our guide to reading firmness labels shows how to compare them.
Zoned support means the coils or foam are firmer under the lower back and hips and softer under the shoulders. A tri-zone mattress has three sections; a seven-zone mattress has finer bands. Zoning matters most for side sleepers, whose shoulders need to sink further than their waist.
What Is the Best Bed Mattress for Back Pain?
The best bed mattress for back pain keeps your spine level in the position you sleep in, which our side, back and stomach positions guide helps you pin down. For most adults, the answer is medium-firm with zoned support. A 2021 review by Caggiari and colleagues in the Journal of Orthopaedics and Traumatology covered 39 studies from 2000 to 2019. It concluded that a medium-firm mattress promotes comfort, sleep quality and spinal alignment. Radwan's 2015 review of 24 controlled trials in Sleep Health found the best results on a medium-firm, custom-inflated mattress. The sleeper set the air firmness.
Both reviews reached their conclusions by firmness, and neither ranked one material above another, so this research does not show foam beating latex or a hybrid beating an innerspring. The material decides how a mattress feels and how long its support lasts. Our foam, hybrid and latex comparison for back pain goes through each build.
Mattress Types Compared for Back Pain
| Mattress type | How it supports you | Back pain notes | One we carry (queen, Oct 9, 2026) |
|---|---|---|---|
| Hybrid (pocket coils plus foam or latex) | Individually wrapped coils, often zoned | Zoning puts firmer coils under the lumbar curve | Ortho Care, tri-zone coils, $695 |
| All-foam | High-density support foam under gel memory foam | Contours the shoulder; check the core suits your weight | Gel Comfort, $1,129 |
| Firm foam | One dense foam core | For heavier sleepers and people told to sleep firm | High Density Firm, 2.5 lb foam, $842 |
| Latex hybrid | Talalay latex over a zoned coil core | Responds faster than memory foam, with no slow sink | Revive St Charles, copper Talalay latex, $3,736 |
| Two-sided (flippable) | The same comfort layers on both faces | Flipping spreads the wear that becomes a sag | Whitney Flippable, tri-zone, $972 |
| Adjustable air | Air chambers you inflate to suit | The best result in Radwan's 2015 review | Not stocked |
Is It Better to Have a Soft or Firm Mattress for Back Pain?
A medium-firm mattress beats both a soft and a very firm one for most people with back pain, going by the strongest trial on the question. In the double-blind Kovacs trial, 313 adults with chronic non-specific low-back pain were randomly given a firm (H(s) 2.3) or medium-firm (H(s) 5.6) mattress. At 90 days, the medium-firm group had 2.36 times the odds of improving on pain in bed. Its odds of improving on disability were 2.10 times higher.
Quoted whole, the published Lancet trial is less tidy. At 90 days, pain on rising (odds ratio 1.93, 95% confidence interval 0.97 to 3.86) was not a statistically clear win. Over the whole study period, the medium-firm group had less pain on rising (p=0.008). Its lead on pain in bed (p=0.064) and daytime back pain (p=0.059) over that period fell short of statistical significance. The trial left out anyone with pain running down a leg, so it tells you nothing about sciatica.
A mattress that is too soft lets the heavier hips sink, bending the lower back into a shallow C. One that is too firm holds the hips and shoulders up, so the lower back loses contact. If you were once told to sleep on something hard, our look at whether a firm mattress is better for your back weighs that older advice.
Body weight and sleep position move the target. Our advisors use the guide below on the Brantford showroom floor. It comes from years of fitting customers, and no study produced it. A 6 on our scale is a general idea and may not feel like a 6 to you.
Back Pain Firmness by Weight and Sleep Position
| Body weight | Sleep position | Firmness to try (10 = firmest) | Why |
|---|---|---|---|
| Under 130 lb | Side | Medium, 5 to 6 | The shoulder and hip need to sink in a little, or pressure builds |
| Under 130 lb | Back or stomach | Medium-firm, about 6 | Too firm and the lower back loses contact and aches |
| 130 to 220 lb | Side | Medium-firm, about 6 | Where most of our back pain customers land |
| 130 to 220 lb | Back | Medium-firm to firm, 6 to 7 | Enough push-back to keep the hips level with the shoulders |
| Over 220 lb | Any | Firm, 7 to 8, with a strong core | Softer beds let the midsection sink and bend the spine |
The C-Shape Test You Can Do at Home
Lie on your back on your current mattress and slide a flat hand under your lower back. If it slips through a big gap, the bed is too firm for you. If you cannot get your hand under at all and your hips feel lower than your ribs, the bed is too soft. Snug, light contact means the support is about right. This thirty-second test tells you more than any firmness label. If you already know you sleep best on a firmer bed, the least expensive mattress in our firm mattress collection is the Spinal Rest at $495 twin.
Identify Your Back Pain Type Before You Buy
Your back pain type changes the mattress, because a bed that suits a sore lower back can be wrong for an aching shoulder blade. A pattern points to a firmness and a build; finding the cause is a job for your doctor or physiotherapist. Each pattern below has its own section further down.
Four Common Back Pain Patterns
- Lower back pain (lumbar region), the most common, often worst in the morning or after sitting.
- Upper back and shoulder pain, more common in side sleepers and desk workers. Pillow size matters here too, as our guide to choosing a pillow explains.
- Morning stiffness that eases once you start moving.
- Hip-referred pain, felt in the lower back but starting at the hip joint.
What the Mattress Research Can and Cannot Tell You
The strongest evidence covers chronic low-back pain with no leg symptoms: the Kovacs Lancet trial (2003) studied 313 such adults for 90 days. The two reviews in this guide add smaller studies, several in people with no back pain at all. Research suggests a supportive medium-firm mattress can ease pain made worse by sleep. HealthLink BC, the B.C. government's health information service, notes that most low back pain gets better within 4 to 6 weeks with basic self-care.
This article is educational and not a substitute for medical advice. Persistent back pain, numbness, or radiating symptoms should be evaluated by a physician, chiropractor, or physiotherapist. No mattress is a treatment for an injury, a disc problem or nerve symptoms.
Best Mattress for Lower Back Pain
The best mattress for lower back pain is medium-firm, roughly 6 to 7 on a 10-point retail scale. Its support layer should stop the hips sinking below the ribs. Lower back pain is the pattern we see most at Mattress Miracle, and the requirements are specific.
- Support layer: zoned coils or dense foam that keeps the hips from sinking too far.
- Lumbar support: some mattresses reinforce the middle section under the lower back.
- Comfort layer: enough cushion to relieve pressure without giving up support.
This guide's pick for lower back pain is the Ortho Care tri-zone hybrid by Sleep In, $695 queen. We chose it because it pairs a 10-inch medium-firm build with tri-zone coils for the shoulders, lumbar and legs, the combination described above. The Spinal Rest is the firmer, lower-cost alternative at $575 queen. If your back is worst the moment you lie down, read why your lower back hurts in bed. Pain low and to one side, near the hip, has its own hip pain mattress guide.
Best Mattress for Upper Back and Shoulder Pain
The best mattress for upper back and shoulder pain is a touch softer than for lower back pain, around 5 to 6 on a 10-point scale. The shoulder has to sink in for the upper spine to stay straight, and side sleepers feel this most.
- Comfort layer: thicker cushioning so the shoulder sinks while the spine stays aligned.
- Material: memory foam or latex comfort layers ease shoulder pressure better than a plain coil top.
At Mattress Miracle, the Oasis Hybrid by Restonit has 3-zone pocket coils, softer at the shoulders and firmer at the lumbar. It is $1,777 queen on sale, down from $2,246. The Gel Comfort all-foam mattress ($1,129 queen) is the softer option, and it ships rolled by courier to Northern Ontario and Western Canada. Pillow height matters here too, and our side sleeper pillow guide walks through loft. The Somnia 4.5 contour pillow ($169.99) is 4.5 inches tall, a common side-sleeper height. For the full breakdown, see our best mattress for upper back pain guide.
Best Mattress for Morning Stiffness and a Sore Back on Waking
A sore back on waking that loosens within 30 to 60 minutes can come from a mattress that sags overnight. It can also come from inflammation, so stiffness that lasts longer, keeps coming back, or comes with pain that wakes you is worth raising with your doctor. In a 2009 Journal of Chiropractic Medicine study, Jacobson, Boolani and Smith worked with 59 healthy adults who had minor sleep-related muscle and joint pain. Each rated back discomfort for 28 nights on their own beds, which averaged 9.5 years old, then for 28 nights on new medium-firm beds. Sleep quality improved, back discomfort went down, and the gains kept building each week.
Morning Stiffness Checklist
- Mattress age: past 8 years, look for a dip you can see or feel. Our article on whether an old mattress causes back pain lists the signs.
- Firmness: medium-firm that does not dip in the middle. The two-sided Whitney Flippable ($972 queen) keeps its support even when you flip and rotate it.
- Bed frame: a sagging frame or broken slats make a sagging mattress worse. Check the centre support, or see our bed frames in stock.
- Pillow: often adds to morning stiffness more than people realize. See our ergonomic pillow guide.
Best Mattress for Hip-Referred Back Pain
Hip-referred back pain is felt in the lower back but starts at the hip joint. At Mattress Miracle we treat its mattress needs as close to lower back pain, leaning a little firmer.
- Firmness: firm to very firm, 7 to 8 on a 10-point scale.
- Side sleeping: zoned support matters most. HealthLink BC suggests lying on your side with a pillow between your knees, as our knee pillow guide explains.
- Avoid: soft memory foam that lets the hip drop deep, which often makes hip-referred pain worse.
The Sentora Ultra Firm ($732 queen) is the budget pick, with 13-gauge pocket coils in seven support zones. The High Density Firm Mattress ($842 queen) is the heavy-duty option: 10 inches of 2.5 lb high-density foam, rated to roughly 300 lb. The extra-firm mattress collection at Mattress Miracle starts at $745 twin.
What the Research Says About Mattresses and Back Pain
Mattress research is thinner than mattress marketing suggests, and what exists points one way: medium-firm over very firm or very soft. These are the four studies this guide relies on, and where each one stops. For the medical side, see our evidence guide to doctor-recommended mattresses.
| Study | Who was tested | What it found | Limits |
|---|---|---|---|
| Kovacs et al., 2003, The Lancet | 313 adults with chronic non-specific low-back pain, no leg pain, 90 days | Medium-firm (H(s) 5.6) beat firm (H(s) 2.3) for pain in bed and disability | Pain on rising not statistically clear at 90 days; over the whole study, pain in bed (p=0.064) fell short; excluded referred pain |
| Jacobson et al., 2009, Journal of Chiropractic Medicine | 59 healthy adults with minor sleep-related pain; own beds averaged 9.5 years | New medium-firm beds improved sleep quality and reduced back discomfort over 28 nights | Small; before-and-after design with no comparison group |
| Radwan et al., 2015, Sleep Health | Review of 24 controlled trials published since 2000 | Medium-firm, custom-inflated mattresses best for comfort, sleep quality and spinal alignment | Firmness was described subjectively in the trials |
| Caggiari et al., 2021, Journal of Orthopaedics and Traumatology | Review of 39 articles from 2000 to 2019 | Medium-firm promotes comfort, sleep quality and spinal alignment | Mixed study designs; materials not ranked |
HealthLink BC's low back pain guide does not name a mattress at all. It says people used to stay in bed, and that doctors now think getting back to normal activities is good for your back in most cases. That matches what we tell people at Mattress Miracle: the bed is one part of the plan. Our guide to what chiropractors look for in a mattress covers the clinician's side.

The Canadian Chiropractic Association lists Sprout mattresses, pillows and duvets, made by Araam Inc., on its endorsement page. Sprout is the only mattress brand the CCA currently endorses. The same page calls it a recognition program and says the funds it raises go to the profession's research programs. We carry Sprout's two-sided natural mattresses, with the medium Durus on our Brantford floor to try, and our Sprout mattress review covers the rest of the line.
Do Adjustable Beds Help Back Pain?
An adjustable bed can help some people with back pain sleep more comfortably, mostly back sleepers who feel better with their knees raised. HealthLink BC's self-care advice includes lying on your back on the floor with a pillow under your knees. An adjustable bed base does something similar in bed, raising the head and knees in the zero-gravity position. The trials in this guide tested mattresses only, so treat a base as a comfort choice and ask your physiotherapist.
Mattress Miracle's adjustable bases start at $349 for the Ergomotion Quest 1.0 in twin XL ($449 queen). The Sleep Beat SE-1005 adjustable base has a German Okin motor and a zero-gravity preset for $1,300 queen. The Deluxe Adjustable Base adds 15 massage programs for $1,550 queen. Our adjustable bed guide covers sizing and set-up.
When to See a Doctor About Back Pain
A mattress can help sleep-related back pain, and it does nothing for the cause of an injury or illness. These signs need medical care before any mattress decision. HealthLink BC's low back pain guide says to call 9-1-1 or other emergency services immediately if:
- Back pain comes with chest pain or other symptoms of a heart attack
- After a car crash, a fall or a direct blow to the spine, you cannot move part of your body
- After an injury like that, you have severe back or neck pain, or weakness, tingling or numbness in your arms or legs
The same guide says to call your doctor now if:
- You have new or worse numbness or weakness in your legs
- You have a new loss of bowel or bladder control
- You have new or increased back pain with fever, painful urination or other signs of a urinary tract infection
- Long-term back pain suddenly gets much worse, and you did not cause it by being more active
- You have a history of cancer or HIV and new or increased back pain
- Pain wakes you from sleep
Some of these signs can come from cauda equina syndrome. HealthLink BC describes it as a serious condition in which the nerve roots at the end of the spinal cord are squeezed, needing immediate medical attention. The NHS back pain page lists these as emergencies. In Canada, call 9-1-1 or go to the nearest emergency department, and do not drive yourself:
- Pain, tingling, weakness or numbness in both legs
- Loss of feeling around your genitals or anus
- Changes in your bladder or bowels, such as difficulty peeing, or peeing or pooing yourself
- Changes in sexual sensation or function
- Back pain that started after a serious accident, such as a car accident
Back Pain Red Flags: See a Doctor Soon
- Sudden severe pain after trauma or injury
- Progressive weakness or numbness in your legs
- Pain radiating down one leg below the knee, which may be sciatica (see our sciatica mattress guide)
- Fever combined with back pain
- Unexplained weight loss
- Pain that does not change with position or movement
- Pain that does not improve after resting, or is worse at night
- A lump or swelling in your back, or a change in its shape
- Pain between your shoulder blades, higher up than the lower back
- Pain no better after 1 or 2 days that stops your normal daily activities (HealthLink BC says to call your doctor)
- Mild to moderate pain still there after 2 weeks of home treatment
- Back pain persisting more than 6 weeks despite conservative treatment
For the signs in the box above, see your family doctor, a physiotherapist or a chiropractor. The Canadian Chiropractic Association's find a chiropractor directory searches its members by province. Bring their advice to Mattress Miracle, and we will match a mattress to it.
Frequently Asked Questions
What is the best mattress for back pain?
For most adults, a medium-firm mattress with zoned support under the lower back. The 2003 Kovacs Lancet trial of 313 adults found medium-firm beat firm for pain in bed and disability at 90 days. This guide's pick at Mattress Miracle in Brantford is the Ortho Care tri-zone hybrid, a medium-firm zoned mattress at $695 queen.
Is a firm or soft mattress better for back pain?
Medium-firm, between the two. Too soft and the hips sink into a C-shape; too firm and the lower back loses contact. The Kovacs trial and the 2021 Caggiari review of 39 studies both point to medium-firm. On most retail scales, that reads as about 6 to 7.
What type of mattress is best for lower back pain?
A medium-firm hybrid or firm-core foam mattress with a reinforced lumbar zone suits most people. Zoning puts firmer support under the hips so the lumbar curve stays level. The research compares firmness levels only, so lie on a mattress before choosing a type. Our mattress firmness guide explains the labels.
What is the best Canadian mattress for lower back pain?
Several Canadian-made mattresses fit the medium-firm, zoned profile the research supports. We carry the Ortho Care by Sleep In ($695 queen) and the Restonit Oasis ($1,777 queen on sale). The Sprout Firmus from Edmonton has 13-gauge coils in its lumbar zone. Labels vary between makers, so try them lying down.
What mattress do chiropractors recommend for back pain?
The Canadian Chiropractic Association endorses one mattress brand, Sprout, made by Araam Inc. Our chiropractor bed recommendation guide covers what clinicians check. Ask your own chiropractor how much lower back support they want, then test mattresses against that.
What type of mattress do doctors recommend for back pain?
HealthLink BC's low back pain guide does not recommend a specific mattress. It focuses on staying active and on positions, such as lying on your side with a pillow between your knees. The trial most often cited, Kovacs 2003, favours medium-firm over firm, so ask your doctor whether your condition changes that.
Can a mattress cause a backache?
A worn or poorly matched mattress can add to a backache, especially stiffness that eases once you move. In the 2009 Jacobson study, 59 adults whose beds averaged 9.5 years old reported less back discomfort on new medium-firm beds. Our article on whether a mattress can cause back pain goes through the signs.
Can a bad mattress cause sciatica?
The NHS sciatica page names a slipped disc as the most common cause, along with spinal stenosis and back injury. None of the studies here show a mattress causing it, and the Kovacs trial excluded leg pain. A sagging bed may make an irritated nerve more uncomfortable, so get leg pain, numbness or weakness checked first.
What's the worst thing you can do for back pain?
Staying in bed for long periods. The NHS advises against it, and HealthLink BC says getting back to normal activities is good for your back in most cases. Take it easy for a day or two, then keep moving gently, such as walking.
What mattress to stay away from?
For back pain, avoid a mattress with a visible dip or sag. Skip a bed so soft your hips sink below your shoulders, or so hard your lower back cannot touch it. Be wary of any mattress you cannot exchange; our comfort guarantee terms give one exchange with no exchange fee.
Can a new mattress really fix back pain?
A new mattress can ease pain made worse by a worn, sagging bed, as the 2009 Jacobson study suggests. It will not cure pain from underlying medical conditions such as disc issues, arthritis or stenosis. If your mattress is over 8 to 10 years old, replacing it is worth trying alongside your doctor's advice.
What bed firmness is best for back pain for heavy sleepers?
Sleepers over about 220 lb usually need firm to very firm, around 7 to 8 on a 10-point scale, with a strong support core. Our firm versus soft mattress guide explains why body weight decides it. The High Density Firm uses 10 inches of 2.5 lb foam rated to roughly 300 lb ($842 queen).
How long does it take for a new mattress to help back pain?
Give it a few weeks. In the Jacobson study, back discomfort and sleep quality kept improving through each week of the 28-night test. Our 60-Night Comfort Guarantee gives you that time. If there is no improvement after 4 weeks on a properly chosen mattress, see a physiotherapist or chiropractor.
