Best Pillow for Neck Pain: Support That Actually Helps

Quick Answer: The best pillow for neck pain keeps your head level with your spine in your usual sleep position. Side sleepers usually need 4 to 6 inches of loft, back sleepers 3 to 4 and stomach sleepers under 3. A 2021 Clinical Biomechanics review found latex and spring pillows eased chronic neck pain, so match the height first, then the fill.

A neck support pillow, also sold as a cervical or orthopaedic pillow, is a pillow shaped or filled to hold the natural forward curve of your neck while you sleep. The right one fills the gap between your head and the mattress. At Mattress Miracle, a family-owned store in Brantford since 1997, we fit that gap in person. You lie on a real mattress and try the cervical and neck support pillows on our showroom floor until your neck sits straight.

Pillow loft is the height a pillow keeps once your head is resting on it. Side sleeping needs the most loft because the shoulder lifts the head off the mattress; back sleeping needs less, and stomach sleeping almost none.

A 2021 systematic review in Clinical Biomechanics by Pang, Tsang and Fu pooled nine high-quality trials with 555 participants. It found rubber (latex) and spring pillows reduced neck pain, waking symptoms and neck disability. It also suggested side-lying alignment depends more on a pillow's shape and height than its fill, which is why Mattress Miracle fits loft first.

Brad Grose, founder, Mattress Miracle, family owned in Brantford since 1997, with 40+ years in the mattress industry: "I will tell you the same thing we tell everyone who walks in with a sore neck. Do not buy the most expensive pillow on the shelf. Buy the one that puts your neck in a straight line when you lie down the way you actually sleep. Bring your worst night in, lie on the bed you would buy, and we will find the height together. That is the whole secret, and it costs you nothing but a visit."

What Type of Pillow Is Best for Neck Pain?

The type of pillow that is best for neck pain, going by the clinical trials, is a supportive latex (rubber) pillow at the right height for your position. A 2009 Manual Therapy study by Gordon, Grimmer-Somers and Trott gave 106 side sleepers a week on each of six pillows. Rubber pillows did best for waking neck pain, sleep quality and comfort. The authors advised against feather pillows and found no advantage for a foam contour shape over regular shapes. Our pillow ergonomics guide explains the neck mechanics behind those results.

A 2025 ChiroUp survey of more than 200 chiropractors found 50.0% most often recommend a contoured cervical pillow, 29.1% memory foam and 13.9% a water-based pillow. For pillows elsewhere in the house, our Canadian pillow buying guide covers every fill and size we sell.

Study Who was tested What it found
Lavin et al., 1997, Arch Phys Med Rehabil 41 people with benign neck pain A water-based pillow lowered morning pain and improved sleep versus a roll pillow and their usual pillow
Persson and Moritz, 1998, J Manipulative Physiol Ther 55 people testing six neck pillows over 3 weeks 36 of 55 slept better; 27 of 42 reported less neck pain
Gordon et al., 2009, Manual Therapy 106 side sleepers Rubber (latex) best; feather "should not be recommended"; foam contour no better than regular shapes
Sacco et al., 2015, J Manipulative Physiol Ther 21 adults without neck pain, pillows 5, 10 and 14 cm high 10 cm (about 4 inches) most comfortable on the side
Vanti et al., 2019, Physical Therapy 70 adults with chronic neck pain A spring pillow reduced neck pain and headache more than education alone
Pang et al., 2021, Clinical Biomechanics 9 high-quality trials, 555 participants Rubber and spring pillows reduced neck pain and disability
Ghosh et al., 2025, Rehabilitación 5 trials, 239 people with chronic neck pain No type (latex, foam or standard) clearly superior

What the Pillow Research Can and Cannot Tell You

Most pillow trials are small, and the 2025 Ghosh review of five trials (239 participants) found no one material clearly ahead. Research suggests the right pillow can ease posture-related neck pain. HealthLink BC, the British Columbia government's health information service, lists a pillow that is too high, too flat or unsupportive among the common causes of neck problems. Medical note: a pillow does not treat injury, disc problems or nerve symptoms. If your pain is severe, follows an accident, or comes with arm numbness or weakness, see your doctor or a physiotherapist first.

Why Your Pillow Causes Neck Pain

A pillow causes neck pain when it holds your neck bent for hours. Your cervical spine (the seven small bones of your neck) has a gentle forward curve. A pillow's job is to keep that curve level with the rest of your spine. Too thin, and your head drops; too thick, and it tips up. HealthLink BC advises checking your pillow and sleep position when neck pain is worse in the morning.

A worn pillow does the same thing slowly. If you wake up sore most mornings, our guide on why you wake up with neck pain every morning walks through the other usual causes, including mattress sag.

Best Pillow for Neck Pain by Sleep Position

The best pillow for neck pain depends on your sleep position, because each position leaves a different gap between your head and the mattress.

Side Sleepers with Neck Pain

Side sleepers need a firm pillow with 4 to 6 inches of loft, higher for broad shoulders. Firm latex, adjustable shredded foam or a tall cervical pillow holds that height all night. Our side sleeper pillow guide goes further into shoulder width.

Back Sleepers with Neck Pain

Back sleepers need a medium pillow of about 3 to 4 inches that supports the neck curve without pushing the chin toward the chest. A contour pillow suits many back sleepers. If a moulded shape feels restrictive, a springy Talalay core that holds its shape gives the same steady height in a regular form. Several of our latex pillows come in low and high profiles.

Stomach Sleepers with Neck Pain

Stomach sleeping is hardest on the neck because the head stays turned for hours, and HealthLink BC advises against it. Until you can switch, use a soft pillow under 3 inches, or none. Combination sleepers do best with an adjustable pillow at 3 to 5 inches.

Talia Grose, sleep specialist, Mattress Miracle: "When someone comes in with morning neck pain, the first thing I check is the gap between their shoulder and their ear when they lie on their side. A broad-shouldered side sleeper needs a much taller pillow than a petite back sleeper, and putting them both on the same trendy contour pillow is exactly how people end up worse. Loft is the whole game."

Side 4 to 6 inches Medium-firm to firm Latex, adjustable shredded foam, tall cervical Orthex Somnia 4.5 ($169.99) or 5.5 ($179.99)
Back 3 to 4 inches Medium Cervical contour, memory foam, latex Orthex Somnia 3.0 ($149.99)
Stomach Under 3 inches Soft Thin soft pillow, or none Fitted by hand in the showroom
Combination 3 to 5 inches Medium Adjustable shredded foam MyWonder adjustable ($49.99)

How to Choose Your Pillow Loft for Neck Pain

Pillow loft is the most important measurement for neck pain. In your usual position, your nose, chin and breastbone should line up, with your head tipped neither up nor down. A 2015 study by Sacco and colleagues in the Journal of Manipulative and Physiological Therapeutics tested foam pillows 5, 10 and 14 cm high on 21 adults without neck pain. Muscle activity was measured lying on the side, and the 10 cm pillow, about 4 inches, was rated most comfortable.

A Simple Loft Decision Framework

  • Start with your position (side 4 to 6 inches, back 3 to 4, stomach under 3), going higher for broad shoulders.
  • A soft mattress lets your shoulder sink, so you need less loft; a firm one needs more. Our mattress guide for neck pain covers the other half of the setup.
  • Have someone photograph you from behind while you lie down. If neck and spine form one straight line, the loft is right.
  • If you switch positions, pick a pillow you can add fill to or remove it from. Removable gel inserts work the same way and draw some heat off your head.

Orthex makes the Somnia cervical pillow, which Mattress Miracle stocks in Brantford, in four heights, per the Orthex 2026-27 catalogue. They are 3 inches for back sleepers, 3.5 for travel, 4.5 for average side sleepers and 5.5 for broad shoulders. Our Somnia pillow height guide shows how we choose between them.

Dorothy Balicki, Mattress Miracle: "The thing people forget is that a new mattress changes the pillow you need. We deliver a firmer bed, the shoulder no longer sinks in like it did, and two weeks later they call about neck pain and blame the mattress. Nine times out of ten it is the old pillow that is now too low for the new bed. We would rather sort the pillow out when we deliver than have anyone uncomfortable."

Are Cervical Pillows Good for Neck Pain?

Cervical pillows can help neck pain when their shape and height match your neck. In a 1998 study by Persson and Moritz at Lund University Hospital in Sweden, 36 of 55 people said neck pillows improved their sleep. In the same study, 27 of 42 said the pillows eased neck pain.

Contour Pillows

A contour pillow has two raised edges of different heights and a dip for the head. Gordon's 2009 trial found no advantage for a foam contour in side sleepers, so treat it as a back-sleeper fit choice.

Cervical Roll Pillows

A cervical roll is a firm bolster that sits under the curve of the neck, and HealthLink BC suggests a small support pillow under the neck when you sleep. The Somnia half-moon cervical roll we carry is 15 by 6 by 3 inches, $54.99, and made for back sleeping.

Water-Based Cervical Pillows

A water-based pillow has a pouch in its base that you fill or drain to change the support. In a 1997 Johns Hopkins study by Lavin and colleagues, 41 people with benign neck pain tried three pillows. They had less morning pain on the water-based pillow than on a roll or their usual pillow. Mattress Miracle does not stock water pillows; our Chiroflow waterbase pillow review covers the design.

Best Fill Types for Neck Pain

Latex is the pillow fill with the strongest trial support for neck pain, and Pang's 2021 meta-analysis favoured rubber pillows for reducing neck pain. It springs back when you roll, so the height stays put. For a softer natural option, the Natura Aloe Dream Mate pairs granulated latex with an aloe-enriched cover. A solid memory foam pillow moulds to your head and neck and holds one fixed height; shredded memory foam lets you change the loft.

Down feels soft but compresses under the head, and Gordon's trial advised against feather pillows for neck pain. We carry goose down pillows for people who sleep well on them, and we point sore necks toward latex or foam first. Buckwheat hulls stay firm, but none of the trials above tested them, and we do not stock them.

What We See on the Showroom Floor in Brantford

Mattress Miracle has fitted pillows alongside mattresses in Brantford since 1997. Most people walk in sure they need a contour pillow, and many do better on an adjustable shredded-foam pillow they can tune to their own shoulders. Contour pillows suit committed back sleepers, but most customers move between side and back through the night, and a fixed shape fights them every time they roll over. Our usual recommendation is an adjustable pillow plus the right firmness underneath it, which our guide to the best mattress for neck and shoulder pain covers.

How to Test if a Pillow Works for Your Neck

Give a new pillow 5 to 7 nights before you judge it, because neck muscles take a few nights to settle at a new height.

The Pillow Test

  • Nights 1 to 3: some discomfort is normal while you adjust.
  • Nights 4 to 7: your neck should start feeling better in the morning.
  • Nights 7 to 14: if pain has not improved or has worsened, the pillow is likely wrong for you.

One store policy matters here. Mattress Miracle's refund policy for pillows and bedding takes returns within 60 days only when the item is unused and in its original packaging, and sale items are final. So try it in the showroom first. If you are unsure your current pillow is the problem, our checklist on how to tell if a pillow is good for neck pain runs through the signs.

How Often Should You Replace a Pillow for Neck Pain?

Most chiropractors recommend replacing a pillow every 1 to 2 years, according to the 2025 ChiroUp survey, and sooner if it loses shape, turns lumpy or leaves you stiff. ChiroUp adds that feather and down can lose loft within months, while contoured and water-based pillows may hold support for three to five years. Fold the pillow in half: if it stays folded, it has lost its support.

When to See a Doctor About Neck Pain

A new pillow can ease posture-related neck pain, and it does nothing for an injury, a pinched nerve or an illness. HealthLink BC advises calling 9-1-1 or other emergency services right away if:

  • Neck pain comes with chest pain or other heart attack symptoms, such as shortness of breath and nausea
  • After a fall, car crash or blow to the spine, you cannot move part of your body
  • After an injury like that, you have severe neck pain, or weakness, tingling or numbness in your arms or legs

It advises calling your doctor now or getting immediate care if:

  • You have new or worse numbness or weakness in your arms or legs
  • You lose control of your bladder or bowels
  • Long-term neck pain suddenly gets much worse and you did not cause it by being more active
  • You have a history of cancer or HIV and new or increased neck pain
  • Neck pain wakes you from sleep

HealthLink BC's guide to neck problems and injuries also notes that a severe headache, stiff neck and fever together can be signs of meningitis, which needs care right away. See your family doctor, a physiotherapist or a chiropractor if pain shoots down your arm, your hand tingles, gripping gets harder or headaches keep getting worse. The Canadian Chiropractic Association lists clinics near you in its find a chiropractor directory. Any neck pain after a fall or car accident needs the same check, as does pain that has not eased after two weeks on a better pillow. Bring their advice to the store, and we will match a pillow to it.

Frequently Asked Questions

What are the best rated pillows for neck pain?

In the research, latex (rubber) and spring pillows rate best: a 2021 Clinical Biomechanics review of nine high-quality trials found both reduced neck pain and disability. Mattress Miracle carries Orthex Somnia cervical pillows from $54.99 for the half-moon roll to $179.99 for the tallest side-sleeper height, and latex pillows from $69.99.

Should I use a cervical pillow for neck pain?

A cervical pillow is worth trying if you sleep on your back and wake up stiff, as long as its height matches your neck. In a 1998 Swedish study, 27 of 42 people said neck pillows eased their neck pain. Side sleepers often do as well on a taller regular or adjustable pillow.

How do you use a neck pillow?

On your back, put the raised edge or cervical roll under the curve of your neck and let your head rest in the lower dip, chin level. On your side, use the taller edge and check that your nose lines up with your breastbone. Give it 5 to 7 nights.

How do I stop my neck from hurting when I sleep?

Match your pillow height to your position, and avoid sleeping on your stomach with your head twisted, as HealthLink BC advises. Our guide to sleep positions and pillows for neck pain covers the position changes. If pain lasts or spreads into your arm, see your doctor.

Can a bad pillow cause headaches?

A pillow at the wrong height can strain your neck, and HealthLink BC lists headache among the symptoms of neck pain. In a 2019 Physical Therapy trial of 70 adults with chronic neck pain, four weeks on a spring pillow reduced headache as well as neck pain. New or worsening headaches need your doctor first.

Is a firm or soft pillow better for a stiff neck?

Research points to both: firm support under the neck with a softer surface for the head. In the 1998 Persson and Moritz study, people wanted a pillow that was soft and not too high, and the best-rated one had two firmer cores supporting the neck curve. A very soft pillow with nothing firm inside lets the head sink and the neck bend. Our firm pillow guide compares the firmer fills.

Can the right pillow fix neck pain on its own?

Sometimes. When neck pain comes from sleep posture and a mismatched pillow, the right loft and fill can ease it. A sagging mattress can undo it. Pain that lasts beyond two weeks, spreads down an arm or comes with numbness needs a doctor or physiotherapist.
