Best Pillow for Side Sleepers in Canada (2026 Guide)

Quick Answer: The best pillow for side sleepers is a medium-firm pillow 4 to 6 inches high that keeps your head level with your spine. Match the height to your shoulder width and your mattress. In a 2009 Manual Therapy study of 106 side sleepers, latex pillows did best for waking neck pain, and the authors advised against feather.

A side sleeper pillow is a pillow tall and firm enough to fill the space between your mattress and the side of your head. It keeps your neck straight while you lie on your shoulder. Two things set the size of that space: the width of your shoulders and how far your shoulder sinks into the mattress.

At Mattress Miracle, a family-owned store in Brantford since 1997, most side sleepers who come in with a stiff neck are sleeping on a pillow that has gone flat. The pillows we stock in Brantford list their height or profile for that reason, so a side sleeper can go straight to the taller ones.

For neck alignment, height and shape seem to matter more than fill. Pang, Tsang and Fu's 2021 review in Clinical Biomechanics, covering nine high-quality trials and 555 participants, found side-lying alignment may not differ between latex and feather pillows. Your mattress sets the other half of the gap, as our firmness guide for side sleepers and others explains.

Brad Grose, founder, Mattress Miracle, family owned in Brantford since 1997, with 40+ years in the mattress industry: "When a side sleeper tells me they wake up with a sore neck, I do not ask what pillow they own first. I ask them to lie down on their usual mattress and I look at the line of their nose. Nine times out of ten the pillow is too flat and their head is drooping toward the bed. A pillow is not a one-size purchase. Get the height right for your shoulder and your mattress, and most of the neck complaints sort themselves out."

Loft means the height a pillow keeps with your head on it, which is often less than its height in the bag. Soft fills give up more height under a head than solid latex or dense memory foam do, so judge loft lying down, in your usual position, on your own mattress.

Bring your shoulder measurement and the name of your mattress to a showroom appointment in Brantford, and we will check the height with you lying down.

What Kind of Pillow Should a Side Sleeper Get?

A side sleeper should get a pillow that holds 4 to 6 inches of height all night, with medium-firm support and a fill that does not pack down. A controlled trial on fill comes from James Cook University in Australia. Gordon, Grimmer-Somers and Trott (Manual Therapy, 2009) had 106 side sleepers try their own pillow and five trial pillows for a week each.

Latex came out ahead on waking neck pain, sleep quality and pillow comfort. Own pillows scored about the same as foam and polyester, contour foam did no better than regular foam, and the authors advised against feather. Their 2010 follow-up in the Journal of Pain Research also ranked latex first and feather last for waking headache and shoulder-blade or arm pain. Six of the seven Natura organic pillows we stock are built on latex.

Study Who was tested What it found for side sleepers
Skarpsno et al., Nature and Science of Sleep, 2017 664 Danish workers wearing motion sensors 54.1% of time in bed spent on the side; side time rose with age and body-mass index
Gordon et al., Manual Therapy, 2009 106 side sleepers, one week per pillow Latex ahead on waking neck pain, sleep quality and comfort; contour foam no better than regular foam; feather not recommended
Gordon et al., Journal of Pain Research, 2010 Side sleepers using one pillow and receiving no neck treatment Latex best for waking headache and shoulder-blade or arm pain; feather produced the most waking symptoms
Pang et al., Clinical Biomechanics, 2021 Nine high-quality studies, 555 participants Latex and spring pillows reduced chronic neck pain; side-lying alignment tied to shape and height, with latex and feather alike
Jiao et al., Sleep and Breathing, 2024 15 adults aged 18 to 30 without neck pain (small pilot) Lowest neck muscle activity, and best comfort, when pillow height matched their shoulder width measure
Lei et al., Healthcare, 2021 Review of pillow-height research since 1997 No firm conclusion yet on one best height for side sleeping

What the Research Can and Cannot Tell You

Two cautions sit beside the Gordon results. Pang's review found latex and feather may hold the neck at much the same side-lying angle. And height is the least settled question: Lei and colleagues' 2021 review in Healthcare found no firm answer yet on the best height for side sleeping. That is why Mattress Miracle measures each person.

This is general information from a mattress store and no substitute for medical advice. If you have persistent neck pain, numbness, tingling, or pain that travels down your arm, see your doctor or a registered health professional before you change your setup. Our sleep science hub collects the studies we lean on across these guides.

Why Side Sleepers Need a Different Pillow

Side sleepers need a different pillow because the shoulder lifts the head further off the mattress than any other position does. It is also the position adults spend the most time in. Skarpsno and colleagues (Nature and Science of Sleep, 2017) fitted motion sensors to 664 Danish workers. In bed, they spent 54.1% of their time on their side, 37.5% on their back and 7.3% on their front.

Underfill the shoulder gap and the neck bends down; overfill it and the head tips up. HealthLink BC, the BC government's health information service, names a pillow that is too high or too flat as one everyday cause of minor neck pain. Back sleepers need less height, so if you switch between back and side, our guide to choosing a pillow by sleep position covers the in-between heights.

What to Look for in a Side Sleeper Pillow

Loft (Height)

Loft matters most for a side sleeper, and most adults land between 4 and 6 inches. Orthex, a Montreal maker, sizes its Canadian-made Somnia pillows, which Mattress Miracle carries, along these lines:

  • Petite side sleepers: 3 inches (Somnia 3.0), or 3.5 inches (Somnia 3.5) for a medium frame
  • Average build, roughly 5'4" to 5'10" and under 200 lbs: 4.5 inches (Somnia 4.5)
  • Over 5'10", over 200 lbs, broad shoulders or a firm mattress: 5.5 inches (Somnia 5.5)

To check a pillow, lie on your side on your own mattress and have someone look from the foot of the bed. Your nose should sit roughly centred between your collarbones. A head tilting toward the mattress means the pillow is too low; toward the ceiling, too high. Most side sleepers end up measuring against the medium-firm mattresses we carry.

Firm or Soft: The Best Firm Pillow for Side Sleepers

Side sleepers do best on medium-firm to firm support. A soft, fluffy pillow feels lovely for a few minutes, then compresses and leaves the neck hanging; one that is too hard presses on the ear. The firmest side sleeper options we carry are solid latex, like the Natura Exquisite, and dense gel memory foam, like the Orthex Somnia 5.5.

Shape and Adjustability

A plain rectangle works when loft and firmness are right. A contour only helps when it fits your body, so we fit pillows from our ergonomic pillow collection with you lying down. An adjustable pillow lets you add or remove fill until the loft matches your measurement. Stacking two thin pillows to get there rarely works, because the top one slides and the height changes every time you move. Shredded memory foam is the usual fill, and the removable inserts in the RiseSleep adjustable gel pillow do the same job ($99.99 on sale, regularly $149.99).

How to Measure Your Loft in Two Minutes

You can measure your side sleeper loft at home with a tape measure and a second person. The Mattress Miracle team runs the same method in the Brantford showroom, and over the phone through our shop from anywhere in Ontario service.

The Shoulder-to-Neck Method

  1. Stand relaxed against a wall, without pulling your shoulders back.
  2. Have someone measure the straight-line distance from the wall to the centre line of your neck. That number, in inches, is your starting loft.
  3. Take off half an inch to an inch for a soft or plush mattress, where the shoulder sinks in. Take nothing off for a firm one.
  4. Lie on your side on a pillow at that height and check the nose line. Adjust by half an inch and check again.

If your number points to one fixed height, the solid cores among the latex pillows we stock hold it all night.

Measuring to the shoulder has some lab support. In a small 2024 pilot in Sleep and Breathing, Jiao and colleagues tested 15 adults aged 18 to 30 without neck pain. Pillow heights were set from each person's shoulder width. Neck muscle activity was lowest and comfort highest at one times that measure, with half and one and a half times both worse. A side sleeper caught between sizes usually leaves with an adjustable pillow from our range.

Talia Grose, sleep specialist, Mattress Miracle: "I run this measurement with people in the showroom all the time, and the part that surprises them is how much the mattress changes the answer. A customer will be sure they need a thick pillow because that is what they have always bought, but once they lie on a plush bed their shoulder sinks and suddenly a medium loft is perfect. Measure on the bed you actually sleep on, not standing in a store."

Best Pillow Fill Types for Side Sleeping

Fill decides how a side sleeper pillow feels, how well it holds height and how long it lasts. The table below sets the fills side by side, with today's prices at Mattress Miracle.

Memory Foam (Solid or Shredded)

Memory foam moulds to the head and neck and spreads the pressure. Solid foam holds one height; shredded foam lets you add or remove fill. Its drawback is heat. The flippable Cool Ice pillow pairs memory foam with a gel side you turn over when it warms up ($60 on sale, regularly $109). A foam pillow is only as good as its height.

Latex

Natural latex is springy, holds its shape and breathes better than memory foam. The latex pillows on the Mattress Miracle floor run from $69.99 to $228 today, and both ends of that range are sale prices. The Natura Exquisite latex pillow is one solid Talalay core with no shredded fill to shift ($158 in queen on sale, regularly $179).

Down and Down Alternative

Down on its own compresses under the head, a weak start for most side sleepers. The Gordon trials tested feather, which finished last, so the case against pure down is indirect. If you love down, pick a full fill: Highland Feather suggests Level 3 of its three fill levels for broader shoulders. Our feather and down pillow collection holds the Miracle Sleep 600 fill power goose down pillow ($209 queen on sale, regularly $299). Down alternative flattens the same way down does.

Buckwheat Hulls

Buckwheat hull pillows are firm, adjustable and airy, but heavy and noisy. We do not stock them, and none of the trials above tested them. Our hotel pillow guide compares the fills hotels choose instead.

Ergonomic and Contour Pillows for Side Sleepers

Ergonomic pillows are shaped to cradle the head and hold up the neck. Side sleepers ask us about three designs.

Cervical Contour Pillows

A cervical contour pillow has a raised edge under the neck and a dip for the head. Its height is fixed, so fit is hit or miss; Orthex gets around that by making the Somnia line in four heights, from 3 to 5.5 inches. The Somnia 3.0 posture pillow is the 3-inch back-sleeper height ($149.99), and the 4.5 and 5.5 are the side-sleeper sizes. Gordon's contour pillow did no better than regular foam, so choose a contour for fit and feel.

Crescent-Shaped Side Sleeper Pillows

Some side sleeper pillows have a crescent or U-shaped cutout along the bottom edge for the shoulder or arm. The Eli & Elm Side Sleeper Pillow is a well-known U-shaped example. Mattress Miracle does not carry cutout pillows, so if you want one, buy where you can return it after a few nights.

Dual-Height Pillows

A dual-height pillow has a higher side for side sleeping and a lower side for back sleeping, so a combination sleeper can switch without swapping pillows. If neck pain outlasts a pillow change, our occipital neuralgia pillow guide covers one cause worth raising with your doctor.

Goose Down Pillows for Side Sleepers at Mattress Miracle

A full goose down pillow fills the shoulder gap with a softer feel than foam, though it needs fluffing and suits no one with a down allergy. Our Highland Feather pillows are made in Toronto with 600 loft white goose down and a 5-year warranty. Level 3, the fill for broad shoulders, costs this much in queen:

How Your Mattress Affects Your Pillow Choice

Mattress firmness changes the pillow height a side sleeper needs, because it decides how far the shoulder sinks. That is why our two-minute mattress quiz asks about firmness first. Use these as starting points, then check the nose line.

Mattress Firmness Shoulder Sink Starting Pillow Loft Somnia Height to Try
Soft / Plush Deep 3 to 4 inches 3.0 or 3.5
Medium Moderate 4 to 5 inches 4.5
Firm Minimal 5 to 6 inches 5.5

If you are buying both, buy the mattress first and choose the pillow after a week on it. Eligible mattresses bought here carry our 60-Night Comfort Guarantee. Our guide to the best mattress for side sleepers covers shoulder and hip relief. The side sleeper topper guide covers softening the bed you own, and our mattress shopping guide covers the rest of the buy.

Dorothy Balicki, Mattress Miracle: "The question we hear most at the counter is some version of, can I just use the pillow I already have on a new bed? Sometimes yes, but if you are moving from an old soft mattress to a firmer one, the pillow that felt fine before will usually feel too flat now. Your shoulder no longer sinks in the same way, so the gap got bigger. We always tell folks to test the pillow on the new bed, not the old habit."

The mattresses in our Brantford showroom are the beds we test pillows on, so you can try both in one visit.

Couples share one mattress but rarely one shoulder width, so each partner may need a different pillow height. Our mattress for couples page covers the bed itself. If one of you snores or wants the head raised, a split king adjustable base we carry lets each side set its own angle.

What Is the Best Pillow for Side Sleepers With Neck and Shoulder Pain?

For side sleepers with neck pain, latex set at the height that keeps the neck level has the best trial record. It led both Gordon trials for waking neck pain, headache and arm pain, and Pang's 2021 review found latex and spring pillows reduced chronic neck pain without changing sleep quality. No study here shows a pillow treating a neck condition, so ask your doctor or physiotherapist about the rest of the plan. Our pillow guide for neck pain goes deeper on cervical designs.

For shoulder pain, the Sleep Foundation notes that people tend to sleep on the pain-free side and that a supportive pillow keeps weight off the shoulders. At Mattress Miracle we also suggest hugging a second pillow so the top arm does not drop forward. A shoulder that hurts every night needs a professional's eye. Our cervical pillow guide sorts neck pillows by sleep position.

When Should a Side Sleeper See a Doctor?

The HealthLink BC guide to neck problems and injuries sorts these by urgency:

  • Call 9-1-1 for neck pain with chest pain or pressure, which can signal a heart problem such as a heart attack. Sweating, shortness of breath and nausea are other warning signs.
  • Call 9-1-1 after a fall, crash or blow to the head or neck with loss of movement or feeling, numbness, tingling, or trouble controlling the arms, legs, bowel or bladder. Do not move the person.
  • Get medical care right away for a severe headache with a stiff neck and a fever, the pattern HealthLink BC describes for meningitis.
  • See your doctor, a physiotherapist or a chiropractor for neck pain that persists or gets worse. The same goes for numbness, tingling or weakness in an arm or hand, or pain that shoots down the arm.
  • If your neck has not eased after a couple of weeks on a better pillow, see a professional before buying another.

What Is the Pillow Trick for Side Sleepers?

The pillow trick for side sleepers is a second pillow between the knees, the setup our knee pillow guide for side sleepers walks through, including who benefits most. Without one, the top leg drops onto the bottom leg and twists the spine. HealthLink BC's low back pain advice suggests lying on your side with a pillow between your knees. The Sleep Foundation adds that it eases pressure on the hips and keeps the knees from collapsing onto each other.

A third pillow to hug rests the top arm. A full-length body pillow does both jobs: the 50-inch Orthex Symbia we carry ($174.99) runs from the upper chest to below the knees on most adults. If your lower back is the real problem, the mattress matters more than any pillow, as our guide to the best mattress for back pain in Canada explains. HealthLink BC says to call your doctor now for back pain with new numbness or weakness in the legs, or a new loss of bowel or bladder control.

When to Replace Your Side Sleeper Pillow

Replace a side sleeper pillow when it no longer holds its height. A flattened pillow is the most common problem we see in Brantford. The Sleep Foundation's general rule is every 1 to 2 years, with these spans by fill:

  • Memory foam: every 2 to 3 years
  • Latex: every 2 to 4 years
  • Down and feather: every 1 to 3 years
  • Down alternative: about every 1 to 2 years
  • Polyester: every 6 months to 2 years
  • Buckwheat: fresh hulls about every 3 years

To test a down or feather pillow, fold it in half and let go. If it does not spring back to its shape, the Sleep Foundation says it is time to replace it. Check solid foam and latex for dents or lost height instead. A worn mattress causes the same alignment problem, as our guide to mattress replacement explains.

Pillow Hygiene for Side Sleepers

A side sleeper's cheek presses sweat and skin oils into the pillow all night, so put a protector under a breathable pillowcase and wash the case along with your sheets. A pillow that stays yellowed, smelly or lumpy after washing is due for replacement. A protector for the mattress does the same job for the bed underneath.

Frequently Asked Questions

What is the best pillow for side sleepers?

The best pillow for side sleepers is usually a medium-firm to firm pillow, 4 to 6 inches high, that holds its height all night. Latex has the best trial record, and adjustable shredded fill is the safest pick if you are unsure of your height. At Mattress Miracle in Brantford, we measure your shoulder first.

How thick should a side sleeper pillow be?

Most side sleepers need 4 to 6 inches: more for broad shoulders on a firm mattress, less for a petite frame on a soft one. Heavier side sleepers sink further, which our heavier sleepers page covers from the mattress side.

Should side sleepers use a firm or soft pillow?

Medium-firm suits most side sleepers, and solid latex or dense memory foam such as the Somnia 5.5 holds height best. Hot sleepers can see our dual-sided cooling pillow review.

What are some good soft pillows for side sleepers?

A good soft pillow for a side sleeper feels soft on top but keeps its height. Two kinds qualify: Highland Feather's goose down pillows at Level 3, the fullest fill, and shredded latex or memory foam you can fluff up. Avoid a flat, soft pillow that lets the head drop.

Can the wrong pillow cause neck pain for side sleepers?

Yes. A pillow at the wrong height holds the neck bent for hours, and HealthLink BC lists an unsupportive pillow among the common causes of minor neck pain. If neck pain persists, or comes with numbness or pain down the arm, see your doctor.

What is a leg pillow for side sleepers?

A leg pillow, or knee pillow, sits between the knees when you sleep on your side, so the top leg cannot pull the hip and lower back out of line. HealthLink BC suggests it for low back pain. The Orthex Symbia 50-inch body pillow does the same job and supports the top arm too.

What is the best pillow for someone who switches between side and stomach sleeping?

Combination side and stomach sleepers need a pillow that squashes flat face-down and lofts back up on the side, so adjustable shredded foam or soft down works best. The MyWonder adjustable shredded memory foam pillow ($49.99 on sale) has removable fill. In a small 1998 study (Persson and Moritz), 37 hospital employees and 18 neck patients tried six neck pillows. They described the ideal as soft and not too high with neck support, yet rated a pillow with two firmer neck cores best. See our best pillows for stomach sleepers guide for the stomach position.

Are wedge pillows or ergonomic pillows useful for side sleepers?

Both have uses. A wedge pillow raises the upper body, with your regular pillow on top. A 2021 BMC Family Practice review (Albarqouni and colleagues) found five head-of-bed elevation trials in 228 adults with reflux. All four that measured symptoms reported improvement, but the authors called the evidence too limited for firm advice, so ask your doctor first. Our Symbia orthopedic wedge pillow comes in four angles ($129.99 to $189.99), and an adjustable bed frame we carry raises the whole head of the mattress instead. Our wedge pillow positioning guide shows the setup. Ergonomic head pillows come down to fit, so try the contour lying on your side before you buy. Our related guide to the best pillow for side sleepers in Canada covers alignment from another angle.

What is the best pillow for back and side sleepers, or all-position sleepers?

Back sleepers need less height than side sleepers: Orthex makes its Somnia 3 inches high for back sleeping and 4.5 inches for an average side sleeper. If you change position through the night, start with a medium-loft, medium-firm pillow around 4 to 5 inches. Shredded memory foam lets you take fill out until it works in every position; a dual-height pillow is the other route.

What is the best pillow according to Consumer Reports?

Consumer Reports says it uses 13 tests to rate pillows, some with human subjects, but its ratings are for members only, so we will not guess at its picks. Its public buying guide does say adjustable pillows tend to perform better in its tests, because you can set them to your needs.
